Scheduling Your First Appointment

The first step in scheduling an appointment with one of our licensed counselors is to complete the form below.

Before your first visit, please sign up for or log in to our Client Portal and complete the required intake documents, including the intake questionnaire, informed consent, HIPAA forms, and insurance acknowledgment.

If you are unable to complete the paperwork before your appointment, you may complete it when you arrive. Please allow some extra time before your first appointment to complete these important forms.
