The first step to schedule an appointment to meet with one of our licensed counselors is to complete the form below. Before your first visit, we EXPECT you to Sign up or log in to the client portal to complete all intake document (Intake questionnaire, Informed consent, HIPAA, Insurance acknowledgement). If you are not able to complete the intake document prior to your appointment, you may do so upon your arrival. Just allow extra time your first appointment for this important paperwork.
